Chairman - FMCA TELEMED

Dr. Imam is Chairman of e-Rad and is responsible for the company's direction, including forming strategic relationships and acquisitions. A pioneer in the fields of teleradiology and telemedicine, he served as Chairman and Founder of American Teleradiology, which merged with Nighthawk and went public in 2006 (NSDQ: NHWK). He holds a BS in Mathematics and Computer Science from Brown, an MD from Brown and MS in Biostatistics from Harvard.

Dr. Imam continues to be instrumental in perfecting radiology workflow solutions that he developed and implemented more than 10 years ago. As a business entrepreneur, he has a vast knowledge including business development, relationship management, mergers, acquisitions and startups. Dr. Imam is also the founder of House Calls of D.C and Advanced Medical Initiatives. All business designed to implement operational efficiencies to end users/patients to gain access to better health care options for the markets they serve.




Dr. Pervaiz Qureshi. MD

Medical Director - FMCA TELEMED

Pervaiz Qureshi is a dedicated and reliable internist serving the communities of Jackson Heights and Brooklyn, NY. Dr. Qureshi attended King Edward Medical University, where he received his medical degree, and completed his residency at the Brooklyn Hospital Center. He also completed a residency in internal medicine at Mayo Hospital. Dr. Qureshi runs his own practice. He treats patients 18 and over for a range of conditions, and he is eager to help the wider community receive the care they deserve. Dr. Qureshi is sensitive to the uniqueness of each patient's situation and adapts his consultations and treatment options appropriately to accommodate individual needs and concerns.




Mildred Scharf Ehrenfeld

Head Of Nurse Practitioner - FMCA TELEMED

She was the Principle Owner of Company NameGreenport Health Care Clinic from Jun 1989 – Jul 1998 where she developed a community based health organization dedicated to making health care available to men, women, teens and children and, to serve as a referral base when needed. Designed, wrote, receive and implemented four primary care initiative grants for the expansion of services to the needy. The team of professionals consist of Pediatric Nurse Practitioners, Obstetric Nurse Practitioners and, Gynecologic Nurse Practitioners. Services included birth control, pap smears, treatment of STDs, stress management, breast examination, prenatal care, pediatric care and, H.I.V. testing and counseling.

She worked as Nurse Practitioner in Hamptons Gynecology and Obstetrics, P.C. from Jun 1992 – Mar 1999 where she practiced full range of patient care which included internal examinations, pap smears and endometrial biopsies and, provided comprehensive gynecological care for women of all ages. Lectured for various organizations as a representative of HGOPC. Developed protocol for the implementation of full admitting and discharge privileges at Eastern Long Island Hospital.

She is the Founder of 448 Griffing Avenue Nurse Practitioner which provides primary care with nurse practitioners with a focus on prevention.

She is the CEO of Nightingale Management Services, LLC from Mar 2012 where developed a healthcare model to provide primary care by nurse practitioners with a focus on prevention. This model will be replicated in retail settings throughout the state of New York. The purpose of these centers is to improve patient options while making health care affordable and easily accessible to all community members.
